For all acute visits, such as:
Acute illness, e.g., influenza, sinusitis, conjunctivitis (pink eye), rash, etc..
Acute injuries

For ongoing concerns such as hypertension, diabetes, high cholesterol, and other chronic conditions.
Patients are typically seen about every 3 months, depending on care needs.
Includes the office visit only. Lab review, medication management outside the appointment, and new or recurring concerns may require additional visits.
Comprehensive exam and health review
Labs, imaging, and immunization recommendations as needed
Follow-up visit to review results, recommendations, and referrals where appropriate
