Chronic Health Concern

Cost Per Visit

  • For ongoing health concerns such as hypertension, diabetes, high cholesterol, and other chronic conditions

  • Most patients are seen approximately every 3 months, depending on their care needs

  • Includes office visit only

  • Does not include routine lab review or medication management outside the appointment

  • Recurring or new concerns may require additional follow-up visits

Membership includes:

  • Annual physical

  • Direct access to physician: messages, phone calls
